Double Eyelid Surgery Healing Stages: When Does the Crease Finally Settle?

the double eyelid surgery healing stages go roughly like this. Swelling and bruising peak in the first week, most of it improves around 7–14 days, and the crease usually settles into a natural look in about 1–3 months. Some people are faster, some are slower.
If you are staring at the mirror thinking "this crease is way too high," you are in very good company. Almost everyone goes through this phase, and it is one of the most common questions our patients ask.
Why the crease looks big and puffy at first
After surgery, the eyelid tissue swells as part of normal healing. That swelling lifts the crease and makes the lid look thicker than planned. The crease itself isn't changing much; the swelling on top of it is slowly leaving.
Double eyelid surgery healing stages, step by step
Week 1: the most swelling and bruising. Eyes may feel tight and you may not open them fully. Rest and follow your wound-care instructions.
Around 7–14 days: stitches are usually removed. Bruising and swelling improve clearly, and many people return to normal daily life.
Around 1–3 months: the remaining mild swelling fades and the crease looks softer and more natural.
After that: the scar keeps maturing and usually becomes less noticeable over time.
These are typical ranges, not deadlines. Skin thickness, eyelid fat, the technique used and aftercare all affect your personal timeline.
What can make healing faster or slower
Your eyelid anatomy
Thicker or fuller eyelids generally take longer to settle than thin eyelids.
Aftercare
Following cold or warm compress advice, sleeping with your head raised and avoiding things that increase swelling all help. For a day-by-day look at swelling, read How Long Does Swelling Last After Double Eyelid Surgery?
Eyelid muscle function
If ptosis (a weak eyelid-lifting muscle) is corrected at the same time, the lids may settle on a different timeline. That is why a proper assessment before surgery matters.
When to call your doctor
Gradual improvement every few days is normal. Contact your surgeon promptly if you notice:
pain, redness or warmth getting worse after the first few days
pus or a fever
blurred or changed vision
bleeding that does not stop
a clear difference between the two creases that persists after several months
